Minnesota Aging Pathways Outreach Services

Resource Specialists and volunteers provide comprehensive and unbiased phone-based and in-person assistance to aging adults, caregivers, service providers, and persons with disabilities who are enrolled in Medicare.

Resource Specialists also coordinate and conduct presentations and outreach activities to increase awareness of the niche areas of the Minnesota Aging Pathways.

Minnesota Aging Pathways is a free statewide service of the Minnesota Board on Aging and Area Agencies on Aging. Minnesota Aging Pathways is the federally designated State Health Insurance Assistance Program (SHIP) for Minnesota and is the place to call for Medicare and health insurance questions. Minnesota Aging Pathways Classes and Presentations

The classes listed below are instructed by Minnesota Aging Pathways-trained specialists and volunteers.

To learn more about each class or presentation, click on the title, and a description will appear.

Minnesota Aging Pathways is the State Health Insurance Assistance Program (SHIP) and Senior Medicare Patrol for Minnesota.  Specialists provide free one-to-one assistance with all Medicare and health insurance issues, and also provide in-depth long-term care options counseling.  Information is objective and neutral.

Medicare can be complicated, but Minnesota Aging Pathways is here to help. Whether you’ve been on Medicare for a few years and just need a refresher, or you’re brand new to Medicare, Medicare 101 will help you understand the basics so you can manage your health care options.

Medicare can be complicated, and it changes from year to year. Come learn about what’s new in Medicare for 2026, so you can choose the best plan for your needs. This presentation is offered in the fall of each year during Medicare Open Enrollment.

We can help with Medicare, find prescription drug cost assistance, complete forms and applications, provide long-term care counseling, find resources in your community, connect people with caregiver support, and much more. Disability Hub Minnesota is a free statewide resource network that helps people who have disabilities solve problems, navigate the health system, and plan for their future. Minnesota Aging Pathways helps older Minnesotans and their caregivers find answers and connect to the services and support they need. Fraud and scams are on the rise, with some scammers specifically targeting older adults. But there are ways you can protect yourself and your personal information. This presentation includes information about fraud prevention, and you’ll learn how to detect and report fraud and scams, so you can keep your personal information and yourself safe.

A qualified health care professional must complete a preadmission screening (PAS) before admission to any Medicaid-certified nursing facility. Minnesota Aging Pathways can also provide resource coordination to help older adults return to a community setting after a nursing home stay or help people living at home who are at risk of losing their independence. This presentation is for professionals who want to learn more about how and when to submit a PAS to Minnesota Aging Pathways for nursing facility admission.

A qualified health care professional must complete a preadmission screening (PAS) before admission to any Medicaid-certified nursing facility. Minnesota Aging Pathways can also provide resource coordination to help older adults return to a community setting after a nursing home stay or help people living at home who are at risk of losing their independence. This presentation is for professionals who want to learn more about how Minnesota Aging Pathways’ PAS and level of care 90-day redetermination processes work, including how and when to submit a referral to Minnesota Aging Pathways.

A qualified health care professional must complete a Preadmission Screening before admission to any Medicaid-certified nursing facility, hospital “swing bed”, or certified boarding care, regardless of the length of stay or payer source. This presentation is for professionals wanting more information about the Preadmission Screening process and how it applies to their work and the consumers they serve. Instruction is provided by trained specialists who conduct Preadmission Screening.

Minnesota Aging Pathways assists older adults to remain in their homes or return to their homes from a facility by providing free in-depth support. Resource coordination and ongoing follow-ups promote long-term success in the community, while offering guidance and support to family members and caregivers.
